A 58-year-old woman presented to the emergency department with the chief complaints of acute back pain. She has a previous history of fall, although she denied any direct trauma to the spine. Immediately after that she developed an acute and severe mid-thoracic pain. Past medical history - unremarkable. Social history:  Alcoholic and smoker (30 units of alcohol a week) and smokes 30 cigarettes per day. Physical examination The pain was central with no radiation.

There was no peripheral stigmata of systemic disease and cardiovascular, respiratory, abdominal and neurological examinations are normal. She has a marked kyphosis and has point tenderness over several thoracic vertebrae. A radiograph of the lady is attached.
